About the Role

We are seeking enthusiastic, compassionate, and reliable Support Workers with strong communication skills to join our team.

In this role, you will provide one-on-one support to participants in the community. You will assist individuals in achieving their personal goals and maintaining independence through daily living support and meaningful community participation.

Key Responsibilities

• Provide one-on-one support in line with each participant’s care plan
• Monitor participant safety and wellbeing and report concerns to your supervisor
• Support participants to access community and social activities
• Maintain accurate progress notes and incident reports
• Work collaboratively with families and allied health professionals
• Assist with personal care and daily living activities as required

Essential Requirements

To be considered for this role, you must have:

• NDIS Worker Screening Clearance
• Working With Children Check (Employee)
• Current First Aid and CPR Certificate
• Current Driver’s Licence and access to a reliable vehicle
• Certificate III or IV in Individual Support, Disability, Aged Care, Home & Community Care, Nursing, or equivalent
• COVID-19 vaccination evidence (if required under current guidelines)
• National Police Check
